Blood level determinations of antiepileptic drugs are not available to the majority of physicians who prescribe these medications for the treatment of epileptic patients, yet rapid accurate methods for quantitative assay of these drugs have been developed. Knowledge of the level of the drug in the blood is essential in the treatment of those patients who develop symptoms and signs of intoxication, show unexpected toxicity on multiple antiepileptic drugs, or totally fail to respond to therapeutic doses of a drug. An objective documentary review of the literature is given for an understanding of the relationship of blood level determinations to the variations in absorption, metabolism, excretion, distribution, degree of clinical effectiveness, drug interaction, and toxic side effects. Comprehensive tables of the blood level values that have been published to date for the major antiepileptic drugs are presented. The development of laboratory methods for the quantitative assay of each antiepileptic drug is reviewed in an historical perspective, leading from bromides to ethosuximide and gravimetric analysis to gas-liquid chromatography. Inclusive tables for methodology are also given.
